Welcome to the Department of Human Services (DHS), Child Welfare Services (CWS), SHAKA Family Connections Screen.

The Family Connections Screen is designed:

1. To help former foster youths who transitioned out from DHS foster care at age 18 search for and reconnect with their biological families; and
2. To help biological families search for and offer resource and support to former foster youths or to children who may be currently in DHS foster care.

Please fill out all the information in the request screen completely. A match will be made with CWS data in SHAKA. If a match is found, the request will be forwarded to the former foster youth at the youth’s email address provided by the youth. It is up to the youth to decide on the next steps. If a match is found for a foster child who is still in DHS foster care the information will be forwarded to the child’s current CWS Social Worker.
